Welcome to the Nettle Creek School Corporation application section of our site. All pertinent documents can be downloaded from the list at left. If you have any additional questions you may call our administrative office at any time. All documents but the W-4 are in Word format. The W-4 document is in PDF format.

In order to be placed on the substitute Teacher List for Nettle Creek School Corporation (Hagerstown Elementary and / or Hagerstown Jr-Sr High School), the following materials must be on file in the administrative office:
1. Completed Application for Employment
2. College Transcript
3. Completed W-4 Form and Copy of Social Security Card
4. Teaching Licenses or Substitute Teaching Certificate* (Applicant must have completed at least 30 college credit hours or at least 2 years of documented, positive work experiences with children as a title I assistant or classroom instructional assistant.)

You should not accept a teaching assignment from either school until these items are in your file at the Administration Office.

To obtain a Substitute Teaching Certificate:
1. Apply for a Limited Criminal History Report ĺ This can be done by Nettle Creek School Corporation via website at not charge to you. Simply fill out and return the request to me.
2. Fill out and sign the Application for Indiana Substitute Teaching Certificate and mail it to the Indiana Department of Education. Remember to include the limited Criminal History report and a cashierÁs check or money order in the amount of $15.00 payable to the state of Indiana.
3. The Substitute Teaching Certificate will be sent to our office by IPSB. I will keep a copy and send the original to you. It is your responsibility to see that all required items are submitted.
